An excellent opportunity is now available for an enthusiastic, experienced diabetes nurse to join our team providing acute and community care to children, young people and their families living with diabetes in North East Essex.
We are looking for a positive, motivated individual to join our team.
The team currently consists of 3.5 WTE PDSN's (including this post), 1.0 WTE Paediatric Diabetes Dietician, 2 Paediatric Consultants with an interest in diabetes and a part time Paediatric Psychologist. We work as a cohesive team with good working relationships and support. The PDSN's and Paediatric Consultants work from the same office which also aids ease of communication.
The CYPD Team look after 185 children and young people up to their 19th birthday. We have a growing insulin pump service, currently averaging 2 pump starts per month. The service provides additional PDSN and Diabetes Dietician support clinics weekly and has recently introduced a home visit clinic in the Tendring area. There is also a joint PDSN and Psychology clinic. We hold regular clinics with the adult diabetes team for those young people aged 16-19 years, these clinics also have support from the generic Transition Nurse Specialist. Joint working between the CYPD and adult teams provide DAFNE courses just for those about to fully transition to adult services.
We encourage team members to develop their own areas of interest, either locally or across the region. For example one of the current PDSN's leads locally on transition and one of the Diabetes Dieticians supports the pilot East of England Sports Clinics.
This is a part time role and the PDSN's work flexibly to accommodate each others preferred working patterns, while ensuring the service is fully covered Monday to Friday 08:00hrs to 17:00hrs.
The successful candidate will be expected to manage their own caseload and work independently in the trust and the community, but with multidisciplinary team support;
Assess and implement the care needs of children with newly diagnosed diabetes and provide ongoing support and education for children and their families in conjunction with MDT
Develop and enhance the child and family's skills and knowledge required for managing diabetes.
Will participate in the East of England Paediatric Diabetes out-of-hours on-call service. The Colchester PDSN's and Paediatric Consultants provide cover for this service 1:5, averaging 1-2 on-calls each in this week. There is and East of England Paediatric Network approved guideline to follow.
Promote and facilitate a continuous support service for children with diabetes, both in hospital and in the community.
Participate in training and updating for ward staff and other HCP's, internal and external to the trust.
